Premature Ejaculation (PE)?  How soon is too soon?

Premature ejaculation (PE) is a common sexual dysfunction characterized by the inability to delay ejaculation during sexual activity, leading to distress or dissatisfaction for both partners.  International guidelines define this as ejaculation within 1 minute of penetrative intercourse. Studies suggest that PE affects approximately 20-30% of men globally, making it one of the most prevalent sexual disorders. While the exact cause of PE remains unclear, several factors are believed to contribute to its development. Psychological factors such as performance anxiety, stress, and relationship issues can play a significant role in premature ejaculation. Additionally, biological factors, including abnormalities in neurotransmitter levels, genetics, and hormonal imbalances, may contribute to the condition. Certain risk factors such as erectile dysfunction, prostate problems, and thyroid disorders have also been linked to an increased likelihood of experiencing PE. Overall, the multifactorial nature of premature ejaculation underscores the importance of comprehensive assessment and tailored treatment approaches to address this prevalent sexual concern effectively.

Curvature causing issues?  Lets talk Peyronie's Disease (PD).

Peyronie's disease is a condition characterized by the development of fibrous scar tissue, or plaques, within the penis, causing curvature or deformity during erections and often resulting in pain or difficulty with intercourse. Named after the French surgeon François de la Peyronie, who first described it in 1743, Peyronie's disease has been recognized for centuries, although its exact cause remains elusive. Prevalence estimates suggest that Peyronie's affects around 1-3% of men, although the true incidence may be higher due to underreporting. While the precise etiology of Peyronie's disease is not fully understood, it is believed to involve a combination of genetic predisposition, penile trauma, and abnormal wound healing processes. Risk factors for developing Peyronie's disease include advancing age, a history of penile injury or surgery, and certain connective tissue disorders. Treatment modalities for Peyronie's disease range from conservative measures such as oral medications and penile traction therapy to more invasive options including injectable medications, shockwave therapy, and surgical procedures like penile plication or grafting. Each treatment approach is tailored to the individual patient's symptoms, severity of curvature, and overall goals for sexual function and satisfaction.

Here at Virtus we are effectively able to educate, provide guidance and offer treatment for Peyronie's disease (new onset disease).  Platelet Rich Plasma injections are coming later in 2024 and we will also be adding Low Intensity Shockwave (LIWST) for treatment of this late and ED in 2024/25 which can also provide options for latent disease or curvatures >30 degrees.

Fertility, the ability to conceive a child, is a crucial aspect of reproductive health for both men and women. Infertility, defined as the inability to conceive after a year of regular, unprotected intercourse, affects approximately 10-15% of couples worldwide. Numerous factors can contribute to infertility, including age, hormonal imbalances, anatomical abnormalities, lifestyle factors (such as smoking and excessive alcohol consumption), and underlying medical conditions like polycystic ovary syndrome (PCOS) or endometriosis in women, and low sperm count or erectile dysfunction in men. Treatment modalities for infertility vary depending on the underlying cause and may include:

  • Medications to stimulate ovulation in women or improve sperm production in men

  • Intrauterine insemination (IUI), where sperm is placed directly into the uterus

​​

  • In vitro fertilization (IVF), involving fertilization of an egg outside the body and implantation into the uterus

​​

  • Surgery to correct anatomical abnormalities or address issues like blocked fallopian tubes or varicoceles in men

​​

  • Assisted reproductive technologies (ART), such as intracytoplasmic sperm injection (ICSI) or egg donation, for couples with severe infertility issues.

Work with us!  We use a discrete process that allows you to provide sampling through the comfort of your own home and sent VIA overnight mailing.  We may not fully be able to help with fertility based on workup.  If this is the case, we can offer you our recommendations and guidance on available resources for IVF, ICSI, and surgical recommendations for Varicocele.

For men considering hormone therapy but still wishing to maintain fertility while on TRT we do offer options to help maintain and improve fertility while staying on therapy.  We also offer options for sperm banking prior to starting therapy should this ultimately be a concern.
